DIRECT CARE WORKER

HOUSE MANAGER

PROGRAM SPECIALIST

TITLE XIX NURSE (R.N. OR L.P.N.)

JOB/LIFE SKILLS/PERSONAL SUPPORT


DIRECT CARE WORKER
Compassionate and motivated people are needed for direct care positions. Responsible for providing individuals with disabilities a productive and independent lifestyle through positive role modeling and assisting with daily living skills.


Full time block shift positions (2 ½ day workweek requiring sleepover) now available. These positions offer a short work week. Shift positions are also available on a full and part-time basis for second and third shift. All full-time positions above 32 hours include benefits.

• PRN (on call/as needed) positions also available.

Positions in some areas require union membership.

Requirements:
• Must be at least 18 years of age
• High school diploma (or equivalent)
• Valid PA driver’s license
• Experience is helpful, but not required
• Able to assist in lifting individuals of various weights

HOUSE MANAGER POSITIONS
The house manager oversees the operations of one (1) community home, while working a scheduled shift, allowing for "hands on" management and providing immediate problem solving. Must be independent and possess good leadership and organizational skills.

Requirements:
• Must be at least 18 years of age
• High school diploma (or equivalent)
• Valid PA driver’s license
• One (1) year work experience with individuals with developmental disabilities
• Supervisory experience preferred
• Ability to lift individuals of various weights
• This is an "on-call" position with pager availability

PROGRAM SPECIALIST
Are you a self-starter, detail oriented? Do you possess good verbal and written communication skills? If so, this position may be the job you’ve been looking for. The Program Specialist along with a team of MH/MR professionals develop training plans for individuals with disabilities, and effectively train staff on implementation and documentation of plans. Also interacts and maintains contact with families, therapist, outside agencies, and others involved in the individuals’ lives. Responsible for monthly, quarterly and annual progress reviews.

Requirements:
• Must be at least 18 years of age
• Valid PA driver’s license
• Master’s degree with one year experience, or
• Bachelor’s degree with two years experience, or
• Associate’s degree with four years experience

TITLE XIX NURSE (R.N. OR L.P.N.)
Are you a Registered or Licensed Practical Nurse seeking a new challenge? Look no further. This position requires a compassionate person to perform clinical nursing duties, oversee medical appointment scheduling as well as assess and monitor needs. The Title XIX Nurse visits the individuals in their community living residences on a monthly basis. The nurse also attends quarterly/annual progress reviews, completes monthly vital statistics and maintains medical forms.

Requirements:
• Must be at least 18 years of age
• Valid PA driver’s license
• Current Pennsylvania nursing license
• Understanding of MH/MR field helpful
• One to two years experience in medical/surgical/geriatric nursing preferred

JOB/LIFE SKILLS/PERSONAL SUPPORT
These are full-time one-on-one positions. Staff is responsible for providing assistance to the individual in all areas of activities of daily living, socialization, recreational and vocational training and employment. The support staff serve as role models to the individuals and must maintain a professional manner at all times.
Requirements:
• Must be at least 18 years of age
• Valid PA driver’s license
• High school diploma or equivalent
• Minimum of one (1) year experience working directly with individuals with developmental disabilities preferred
• Able to assist in lifting individuals of various weights
• Effective verbal and written communication skills
• Ability to work independently
